Copyright Consent Form

I acknowledge that Concrete Schoolyard has received a request from a client to produce a custom printed product that utilises an artwork or image for which I am the copyright holder.I agree that Concrete Schoolyard has the right of reproduction of the artwork or image described and provided below, for which I am the copyright holder, for the fulfilment of the custom printed product order in question.

I acknowledge that Concrete Schoolyard will not reproduce the artwork or image in the future without first requiring repeated permission.

I acknowledge that Concrete Schoolyard will be depending on the accuracy of the statements and representations I provide in this release as a condition for agreeing to print/reproduce the image or artwork.

I hereby warrant and represent to Concrete Schoolyard that:

I am the copyright holder of the artwork or image described and uploaded below,and
I provide permission for Concrete Schoolyard to reproduce the artwork or image for the production of a custom printed product for the client in question.

I agree to defend Concrete Schoolyard harmless from any loss, demand, suit, damages, claim or action resulting from my representation, use or reproduction of the artwork or image submitted for the order in question.

I agree to pay any cost or fees (including reasonable attorney fees) incurred by Concrete Schoolyard in connection with any loss, demand, suit, damages, claim or action resulting from my representation, use or reproduction of the artwork or image(s) for the order in question.I acknowledge that Concrete Schoolyard may contact me requiring further information or confirmation via the email address given below to avoid false representation.
